Home and Community-Based Services (HCBS) provide opportunities for Medicaid beneficiaries with behavioral health conditions to receive services in their own home or community.

Psychosocial Rehabilitation (PSR) – is a collaborative person-directed, and individualized. These services are recommended by your therapist and can be provided at home. It is a part of a wrap-around treatment plan. The focus is on helping individuals develop skills and access resources needed to increase their capacity to be successful and satisfied in the living, working, learning and social environments of their choice.

Habilitation services – may help individuals develop skills necessary for community living and recovery with ongoing assessment of individuals’ functional status and development of rehabilitative goals.

Peer and Family Support Services- these services provide support from people who have been successful in the recovery process who now help others experiencing similar situations. Also, with Family Support Services the structure reflects a strength-based relationship with rehabilitation and recovery focus. Activities included must be intended to achieve the identified goals or objectives as set forth in the participant’s individualized recovery plan.
