Our mission is to make physical activity and exercise a standard part of global disease prevention and treatment medical paradigm. Our staff and programs are tailored to serve your specific needs and help you achieve your health goals. A SILVER SNEAKERS SERVICE PROVIDER:
Silver Sneakers is a health and fitness program designed for adults 65+ that’s included with many Medicare Plans. However, any individuals with a Medicare Plan can check their Silver Sneakers eligibility online or with the support of EE Pathways.

Health and Recovery Plans (HARPs) are Medicaid Managed Care plans that pay for your physical and behavioral health care.

Home and Community-Based Services (HCBS)
We provide opportunities for Medicaid beneficiaries with behavioral health conditions to receive services in their own home or community.
Community Oriented Recovery and Empowerment (CORE)
CORE Services provide opportunities for eligible adult Medicaid beneficiaries with serious mental illness and/or substance use disorders to receive services in their own home or community.
